The Gateway to Adult Services
Your local aging and disability resource center (ADRC) is the first place to go to get accurate, unbiased information on all aspects of life related to aging or living with a disability. The ADRC provides information on a broad range of programs and services, helps people understand the various long-term care options available to them, helps people apply for programs and benefits, and serves as the access point for publicly funded long-term care. Family Care
Family Care is a long-term care program that helps frail elders and adults with disabilities get the services they need to remain in their homes.
To understand Family Care, it helps to know what “long-term care” is. Long-term care is any service or support that a person may need because of a disability, getting older, or having a chronic illness that limits their ability to do the things that are part of their daily routine. This includes things, such as bathing, getting dressed, making meals, going to work, and paying bills. Read more
When a person decides to enroll in Family Care, they become a member of a managed care organization. Managed care organizations operate the Family Care program and provide or coordinate services in the Family Care benefit package.

IRIS
What is the IRIS program?
Include, Respect, I Self-Direct (IRIS) is a self-directed program for Wisconsin’s frail elders and adults with disabilities. You have to be eligible for Medicaid to be in the IRIS program.
IRIS is built on the principles of self-determination and self-direction. That means that you will have the freedom to decide how you want to live your life.
When you join IRIS, a budget is established for you based on your long-term care needs and goals. You get to decide what goods, supports, and services will help you achieve your goals and, using your budget, you will help create a support and service plan to meet your needs.
IRIS consultant agencies oversee and support consultants in your area. The IRIS consultant agency you choose will send you informatoin about the consultants who are available in your area. IRIS consultants work with consumers to develop one’s IRIS plan. Everyone in IRIS will get help from a fiscal employer agent. After you enroll, your IRIS consultant agency will offer you options and you will choose which fiscal employer agent you want to work with. The fiscal employer agent does background checks on workers you hire, processes payroll, manages the amount of federal and state taxes to take out of your workers’ checks, and helps with other employer tasks. The fiscal employer agent also processes any cost share you may owe and pays vendors for services.
